Dear Patient,
Thank you in advance for completing this short survey. Your views will help our practice understand how our appointment system can be improved, if at all.
From Monday 16th January 2012, the Surgery will be starting a six month trial of longer appointment slots for patients seeing Drs Lodge, Fielding and Hodgson to see what effect this has on patient waiting times. This means that the last appointment to be offered will be at 5.30pm. The Surgery will also be closing at the slightly earlier time of 6pm but we are continuing our morning clinic on Saturdays from 8.30am until 10.30am.
Please answer all of the questions and click 'Send Survey' when you are done.
Q1. How easy do you find getting into the building at the surgery ?
Q2. How clean is the GP surgery ?
Q3. In the Reception Area, can other patients overhear what you say to the receptionist?
Q4. How helpful do you find the receptionists at the surgery ?
Q5. How long after your appointment time do you normally wait to be seen?
Q6. How do you feel about how long you normally have to wait ?
Q7. Did you know that we are open on Saturday mornings from 8.30am until 10.30am?
Q8. If you are currently unable to get to the Surgery between 9am and 5pm, when would you prefer to see a Doctor?
To help us analyse your answers please tell us a few things about yourself:
Are you male or female?
What age are you?
What is the ethnic background with which you most identify?
How would you describe how often you come to the practice?
Many thanks for your time in answering the questions on this survey. Once we have received all of the results then we will analyse the findings and report back to you in February.
