The Castle Medical Centre has recently set up a Patient Reference Group. The aim of the group is to involve our patients in decisions about the range and quality of services provided and over time commissioned by the practice.

In order to prioritise areas for development the Patient reference group would like to be able to get opinions from as many patients as possible and we are asking if you could spare a few minutes to complete this short questionnaire so that your views can also be considered.
