Dear Patient,

We want to know what we do well and where we could improve our services for you. As ever, please limit your suggestions for improvement to issues within the practice's control rather than those that would rely on different parts of the wider NHS changing.

The survey will run from 22 September 2018, through to 30 November 2018 and members of the Patient Participation Group (PPG) will approach a cross section of patients to obtain their feedback. We will then analyse the results and publish these on our website. Thank you very much for your time and your feedback.

SECTION 1: Access to appointments and services

1 When you telephone the practice, how easy is it to get through to the practice on the phone?

20 Frequently our doctors see patients who when asked "how are you" are met with "I am sorry to bother you, I'm not sure if I need to see you, but.." How many of your appointments with a doctor could be managed by someone other than a doctor? For example: a nurse, pharmacist, paramedic practitioner, self-help advice.


SECTION 6: About opening times

21 The practice is currently open Monday to Friday between 8.30am and 6.00pm, in addition we have been collaborating with the Burgess Hill practices to provide extended access for pre-booked, face to face appointments between 6.30- 8pm weekdays and 8am -12noon on Saturday's. Have we been able to meet your needs with these opening times?
