Dear Patient,

As the Patient Participation Group for your surgery, we need your input to help us assist the Practice to provide the best care that is possible to patients in the face of continually increasing demand and tight financial constraints. By completing this questionnaire and voicing your suggestions, you can help us to work with the Practice to identify what matters most to you as a patient and how best we can improve areas identified as priorities.

A. Web Site

How easy do you find our website to use?

How could we improve the website to be more user friendly?

C. Dispensary

You may be eligible to have your medications dispensed at the surgery if you live more than one mile from the nearest pharmacy as the crow flies. Please ask at Reception if you would like to know more about this service. For those patients who use our Dispensing Service, how satisfied are you with the service?

Please write any comments as to how we could improve this service

D. Appointments

The surgery offers appointments outside of normal hours on Monday evenings until 7.15pm, Thursday mornings from 7.00am and one Saturday per month from 7.30am. Did you know that we also offer extended access to appointments at Mowbray Square in Harrogate during evenings through the week, and on a morning at week ends?

E. Waiting Times

Due to the increasing complexity of illnesses, multiple symptoms and frequent emergencies, the surgery is aware that waiting times in the Waiting Room have increased and this can be frustrating for patients. How long do you think it is acceptable to wait to be called in for your appointment in these circumstances?

Is it more important to you that;

The GP takes as longs as it is necessary to address patient concerns (which may exceed 20 minutes)
