1: In coming months the practice plans to make changes to the way in which patients are called in for an annual review. This is in response to feedback that our current system can sometimes mean patients need to make multiple visits for reviews during the year and patients would much rather attend the practice for one complete review annually. 5: The practice is working with the Sheffield Clinical Commissioning Group on a new way of processing patient requests for repeat prescriptions via telephone. This has allowed us to increase the length of time the service is operational from 10am-12pm to 9am-3pm daily. Have you requested a repeat prescription via telephone (via the new Prescription Order Line)?

7: The practice is able to communicate with the pharmacy of your choice to offer an Electronic Prescription Service- this reduces the need to come into surgery to collect your prescription as this will be sent automatically to the chemist. Are you already signed up to this service?

9: Through the practice website patients are able to login to their personal account to book appointments and order repeat prescriptions. Are you signed up to this service?

11: The practice has a Patient Participation Group –this group of patients helps and guides the practice to ensure we make the right decisions and changes to meet the needs of our patients. The group meet four times throughout the year and liaise via email. The notes following each meeting are available on the practice website for all to view. The group also publish a bi-annual newsletter updating patients on practice news. Are you aware of how you can contact this group in order to pass on your ideas and suggestions? 12: The practice is committed to provide high quality care to all of our patients and find both compliments and constructive criticism very helpful in order to make positive changes and improvements.
