Which of the two sites under consideration do you prefer?
Are you happy for your name and address to be used in support of the surgery's search for new premises?
If so, we may write to Richmond Council, individual councillors, the Richmond Clinical Commissioning Group, NHS England, or other political groups including you in a list of names supporting our new premises and calling on them to act accordingly. Please make sure you enter your name and address on the next question if you are happy for us to use these.
If you are happy for us to use your name and address in support of our premises search, please enter these below. You may enter multiple family members' names with their consent.
