Question 2a We run a triage system every morning, which is a system where if you need an appointment before the next available routine appointment then you can leave a message and a Doctor or Nurse will call you back. Have you ever used this service

Question 3a We have implemented the Patient Access service for our patients. This allows you to use online services that we provide, such as arranging appointments, ordering repeat medication and limited access to your medical record.

Question 4 In order to maintain accurate contact details for our patients we are looking at ways in which you can contact us to update any changes to your details, such as a new telephone number or change of address. What do you think is the best way for you to do this?

 
 
 
 

Question 5a We have been providing extended hours clinics for some years now, whereby we offer appointments to patients outside of our normal working hours. These are usually provided on evenings or Saturdays. Have you ever used an extended hours appointment?
