Oak House Surgery - Branch Relocation 2016

Oak House Surgery is proposing to relocate to Ormskirk House Surgery from April 2016.The services currently provided by Dr’s Filletti, Sword and Chen from Oak House Surgery will be continued at Ormskirk House Surgery. The services provided by Ormskirk House Surgery for current patients of that practice will remain in place.

The views of our patients are important to us and will play an integral part in the relocation process. The planned relocation will allow the practice to thrive and provide the facilities and services that ALL our patients deserve. We understand that this relocation may cause some initial difficulties for some patients, however, we are well aware that the practice premises at Oak House Surgery are ageing and are not fit for current standards for the delivery of good quality and safe care to patients.

Every member of staff is committed to making healthcare for our patients the best it can be, and it is our combined view that the relocation will be positive for patient care. A frequently asked questions document is available at both surgeries which will explain how this may affect you.We understand there may be concerns or ideas on how it can best work and we would like to encourage our patients to let us know via this survey.
