CQC Inspection Survey May 2015

As part of our CQC (Care Quality Commission) Inspection on 12 May 2015 the inspectors have asked that they hear directly from members of the Practice Evaluation Group about what you think about our service - both good and bad.

The inspector has asked that members of the group send e-mails directly to him at

tim.price@cqc.org.uk

focusing on your experiences in the past year and to think about:

Were staff caring and did they treat you with dignity and respect?

Was the environment safe and hygenic?

Were you listened to?

Were your needs responded to with the right care and treatment at the right time?

Do you have any other comments about the service we offer?

However we list each question below if you prefer not to send an e-mail response to him although we would urge you to do that please.

Many thanks for taking part in this and providing feedback to the CQC
