This year there has been a lot in the news about difficulty in accessing healthcare whether that be in general practice or at A&E. The governments own survey has revealed variations in satisfaction with services in different parts of the country and in different age groups,
We are interested in what you think about our surgery. The following questions mirror the government survey, and at the end there are a few questions specific to our surgery. Please remember that the new Friends and Family Test is now up and running. You can feed back your experiences at the surgery at any time through the year. Feedback forms are at reception. You can also feed back to us via the automated patient check-in and via the website at www.shaylane.org.
You can do it by phone, letter or email.
As usual we are extremely grateful to all members of our Patient Participation Group for helping to shape our local services.

Please answer ALL the questions you can. There are no right or wrong answers and your doctor will NOT be able to identify your individual answers. Thank you.
