Patient Participation Group Survey
Thank you for agreeing to join the Camberley Health Centre Patient Participation Group. The aim of this questionnaire is to clarify issues that have been raised, establish whether you are aware of changes that have happened in the practice, and to give you the opportunity to raise issues.
There are 14 questions most of which are yes/no or multiple choice so this should not take long to complete but will provide us with a much clearer view of how patients perceive the surgery and allow us to plan for the future taking your views into account.
1. Late Surgery until 8pm

Are you aware that we now have doctors, nurses
and a phlebotomist working late with surgeries running until 8pm?
b.If YES, have you used the service?
i. Why did you use it?
ii. How likely are you to use the service again?

If NO, if you need an appointment would you
choose an appointment between 6.30 and 8pm in preference to an earlier
appointment?

What types of appointment would you prefer to
see in the evenings?
2. Appointments
We currently try to manage appointments by
having a phased release which allows some to be booked weeks ahead but retains
some for patients who need an urgent appointment on the day.
Note that the more appointments that are embargoed the more difficult it would be to book an appointment for a time in the future e.g. to return to the doctor for a follow up, conversely it would be easier to get
appointment on the day if you needed one.
Also note that the greater the proportion of appointments bookable in advance, the more likely it
is that you would not be able to get an appointment on the day - even if
you really needed it.

3. Friends and Family Test
The Friends and Family Test is a new test which
we trialled during the flu clinics and while the results were very pleasing one
of the areas of where there was some concern was ease of telephoning the
surgery. In order to clarify the issues

Busy Times
Are you
aware that the busiest times in the surgery are early mornings, particularly on Mondays or the day after a Bank Holiday, and this applies particularly to telephoning the surgery?

only which would mean that you would have to book a specific time slot in
advance. Do you feel this would be a
benefit to you or restrict you?
