Patient Representation Group Questionnaire

This questionnaire is designed to help improve the service offered by the practice for our patients.

As a result of our last patient survey we have amended our opening hours to 08:00 – 18:00 Monday to Friday. We also run extended hour clinics on a Monday until 19:30 and on a Wednesday from 07:30.
