Dear Member,

It is now three years since we started our virtual Patient Participation Group. I would like to take this opportunity to thank you for your involvement and support in suggesting new ideas.

There have been many changes within General Practice in the past year. Clinical Commissioning Groups have taken over the commissioning of local services, with GP's being more involved in the formulation of referral pathways and local services.

We would like to move forward with our Patient Participation Group and it has been suggested that we elect a committee to consolidate ideas and suggestions from patients about the services we provide, and ideas for improvements in the future.

I would be very grateful for your feedback and will consider any suggestions and/or comments for the upcoming patient survey.

Q1. Is there anything about the service we provide that you feel needs improvement.

Q2. Is there anything that we are currently not doing that you would like to see introduced.

Q3. Would you be interested in forming part of a Committee for the Patient Participation Group?

Q4. Is there anything else that you would like to raise at this time?

I will be distributing a survery for the whole practice population over the next few weeks based on these comments, so a prompt reply would be very much appreciated.

Thank you for your time.
