Dear Medwyn Surgery Patient

Following consultation with the Patient Participation Group and staff we have devised this survey to help us review and improve the services we offer. This is to help us achieve our aim of delivering the highest quality of service and care to you our patients, as your opinions are very valuable.

We would be very grateful if you would take the time to complete the survey based on your experiences at the surgery. Please answer ALL the questions that apply to you. There are no right or wrong answers and we will not be able to identify your individual answers.

Clinical Services
