Patient Matters: District Nursing Team Questionnaire (January 2013)

The aim of this questionnaire is to get patient feedback on whether certain groups of patients currently being supported by the District Nursing Team can be supported in a more appropriate way. This feedback will be passed to the Northern Ryedale Neighbourhood Care Team which is made up of Ampleforth, Helmsley, Terrington, Pickering and Kirkbymoorside Surgeries together with the York Foundation Trust District Nursing Team.)

Please note that these are general statements only and there may well be some patients who - even with support - are not able to carry out these procedures and so will continue to need support from the District Nursing Team.

1.1 Patients who are housebound and need eye drops should be show how to do this for themselves rather than having regular home visits from the District Nursing Team to do this for them.
