Patient Survey - Feb 2013
Thank you for taking part in the Lennard Surgery Patient Survey.
It has been an extremely busy year in the surgery and demand for our doctor and nurse services have reached the highest levels that we have ever experienced. I hope you have seen some of the improvements that we put in place following last year's survey - some of the questions below ask about the impact of those changes.
I would be grateful if you could take the time to answer all of the questions and I look forward to reading your comments regarding the future. This will enable me to focus on the issues that are important in 2013.
Many thanks
