GREEN STREET CLINIC Patient Participation Group (PPG)

The Patient Participation Group (PPG) is a forum organised by the Practice to give patients a voice, provide feedback, positive suggestions, and contribute to decision-making, communication and consultation. It will also monitor services and includes Patient and Practice representatives having been meeting monthly since last autumn 2011. The Green Street Patient Participation Group has been running for a year now, and one of the first things it did was to organise a Patient Survey, the results and actions of which have been published in our Newsletters and on our website www.greenstreetclinic.co.uk We are most grateful to everyone who has taken part.

The Group is very keen to reflect the views and concerns of the whole patient population, and so we would be grateful for your co-operation in completing another short survey to help us direct our future efforts. Any information you provide will be completely confidential and anonymous, although we would ask you to provide a few personal details about your age etc.
