This questionnaire is looking at the service we provide in the health centre. The questions are focused on the areas suggested by the practice’s Patient Participation Group (PPG). The PPG is a representative group of patients at The Leys Health Centre who have told us about the healthcare topics of most interest to them.

The questionnaire should only about take about 5 minutes to complete. Once the replies have been analysed, we will update the practice website and put up information in the surgery about the results and next steps.

Part 2: Access to GPs
