The Patient Participation Group and the Partners of Walnut Tree Health Centre would be very grateful if you could take a few minutes to respond to the questions below. We wish to gain feedback from our patients regarding GP and nurse Prescriber services within the practice. The results of the survey will be published during March 2012.

4 How was the doctor at listening and giving you a chance to describe all your symptoms and problem(s)?
5 How well did the doctor involve you in decisions about your treatment?
6 How well did you feel the doctor treated you with care and concern?
7 Did you feel the doctor took your problems seriously?
8 Do you have confidence and trust in the doctor?
9 If you were not satisfied with the consultation with the doctor you last saw have you made another appointment to see another doctor?
10 If you were still dissatisfied with the decision of the second doctor would you book another appointment with a further doctor?
