COURTENAY HOUSE SURGERY

PATIENT PARTICIPATION GROUP

SURVEY/PATIENT INFORMATION

SPRING 2017

Courtenay House Surgery’s Patient Participation Group is carrying out a survey to support the practice and seek ways to identify patient-practice issues, and support the practice in seeking ways to address these issues with the ever increasing pressures within the National Health Service.

The PPG is made up of patients and is independent of the practice and the NHS. It is your voice so your views are important to us. We are grateful to you for taking the time to complete this short survey and we appreciate any comments whether these are positive or negative as these will help practice staff to improve the service they give you. There is a patient information sheet for this survey this is available as a separate document under the Patient Participation tab, which we hope you will find useful. Results of the survey will be published on the website, hardcopies will also be available at reception. The survey will run until the 31st March 2017.
