Dear Patient,

Once again we are running a patient survey to help us improve our services. All surveys are anonymous. Survey will run until 31st October 2016. You will be able to review the results on our website after that date.

An important part of this process requires us to understand patient access and awareness of the services we already offer and those we hope to offer in the future. In particular, we are running involved in a funded pilot scheme to trial e-consult (Web GP). This can be accessed via our website and provides online non-urgent advice and consultations. The service also provides many self help guides and videos. This scheme will be running for 12 months and if successful, West Hampshire Clinical Commissioning Group may offer ongoing funding to keep this service running.

Smoking: If you are a smoker and would like help to quit, you can access a quit smoking programme at The Village Pharmacy in Fair Oak and Wainwright's Chemist in Bishopstoke. Many other pharmacies offer a quit smoking programme. Details of local services are available on our website or via reception.

Get Walking: Many research projects have shown that there are great health and social benefits to be gained by taking regular walks. We will have a representative from Health Works in the surgery on flu day promoting the benefits of walking and how you can take part in local organised walks. Details will also be available on our website shortly.

Friends and Family Test: You are invited to complete one of these every time you attend or make contact with the surgery, including your visit for the flu vaccination. You can do this online (via our website) or on paper - available from reception.

Online Appointment Bookings/requesting repeat medications

Are you aware that we offer online appointment booking for most of our general practitioners. We hope to extend this booking service to some nurse and blood test appointments in the future. You can also check and cancel bookings via the online service. This online service for both appointments and medication ordering can be accessed via one login and password.
