Integrated Care Communities

Our practice is working with Cumbria Clinical Commissioning Group in developing Integrated Care Communities (ICCs). This is a way of bringing together everyone in an area who is working with the community (from health, councils, voluntary groups and the community itself) to ensure services meet the needs of the local population. To assist with this we want to establish what each community needs to keep well and what health related issues each community feels that they have.It would be helpful if you could spare a few minutes to complete the following to give us an idea of the some of the issues that concern you and also to tell us about the local services that work well.Which age group do you fall into?

What do you think are the most important health related issues that your community has?For example things like difficulties with accessing hospital services, isolation, home care etc.

Do you have any examples of a service in your community which works well?This could be things like a local walking group, an exercise class or voluntary transport service.

A number of local communities in Morecambe Bay are looking at citizen-led projects to improve their health and wellbeing e.g. “run a mile a day” for local school children and designing leaflets to support people to take good care of their health and wellbeing. Is this something you believe your community would be interested in?
