Dear Patient,

Many thanks for agreeing to take this short survey to help us understand how we can improve our services. All surveys are anonymous. Survey will run until 31st October 2013. You will be able to review the results on our website after that date.

As part of a review by NHS England, practices are being asked to show that services we offer, outside our core contract, are valued by the patients and worth keeping.

For our practice, these services include having a nurse practitioner and practice nurses available to see patients with minor injuries or illnesses and therefore avoid the need to travel to A&E in Winchester or Southampton. We also employ a medicines adviser who deals with day to day prescription enquiries and also help patients on multiple medications, including doing home visits to assist our housebound or elderly patients. We now require some feedback from patients to back up our claim that these services are vital to delivering a good service and would be missed if they were cut. A survey will be appearing on the website shortly and in paper form in our waiting rooms.

Nurse Practitioner & Duty Nurse

We have a nurse practitoner four days a week, seeing patients with a range of minor illnesses and injuries. She will assess, treat and prescribe as required as well as providing telephone advice. We also have a duty nurse every day seeing patients with acute problems as well as those requiring urgent review. The duty nurse also carries out nurse triage duties.

Our Medicines adviser works full time and offers help and advice to patients seen in Stokewood and Old Anchor Surgery, liaises with the local pharmacies and nursing homes as well visiting housebound patients to assist them sorting out their requirements.
