Alvanley Family Practice Patient Survey 2014-2015
Dear Patient
Over the last 12 months there have been changes within the practice. Dr Sarah Davies joined us as a salaried GP and Dr Christine Beeston as a regular doctor at the practice. We have had a new practice manager, Miss Kay Ellermeyer and we have changed our clinical system to EMIS which is the most popular in the country. All these changes have had their own difficulties and as a practice we appreciate the support of our patients throughout.
As part of our on-going commitment to improve and develop the practice for all our patients, we would be very grateful if you would complete this survey about Alvanley Family Practice.
We recently ran a mini survey of the priority areas that patients would like us to focus on. We are always looking for more people to join our 'virtual' patient reference group, whereby patients can share their views with the practice via email discussion. If you are not already part of this group and would like to join, please leave your email address and name in the box in question 20a.

1. Thinking about the last time you tried to see any doctor or nurse fairly quickly, were you able to get an appointment either on the same day or within the next 2 weekdays.

8. In response to last years' survey the practice increased it's opening times by 10.5 hours per week. Our current opening hours are Monday 08:00 - 20:00 Tuesday 08:00 - 18:30 Wednesday 08:00 - 18:30 Thursday 08:00 - 18:30 Friday 08:00 - 18:30 Weekend closed. Are you happy with these times?
9. Our reception and adminstrative staff have a challenging and busy job dealing with all patient, hospital and GP queries. They often have to deal with conflicting priorities based on the needs of our patients. How would you rate the service provided by them?
10. At your last appointment, how good was the clinician at:-

11. Changing computer systems has meant we have temporarily lost some of the functions eg being able to book appointments online and request prescriptions. We will be re-instating these fully in 2015. If you used these services previously how would you rate them?

13. One of the ways we convey information to patients is via a powerpoint presentation that runs continually in the waiting room on the calling screen. We update this as often as we can with current health information, including information such as the recent Care Data details. Is there anything else you would like to see on this presentation?
