GP Practice Survey – Have Your Say (2014)

We would be grateful if you could answer some questions about your general practice. This survey has been developed with the Patient Participation Group (The Mote PRG) for the St Saviours Road and Loose practice. We want to provide the highest standard of care, and feedback to this short survey will help us identify areas that could be improved. The findings of the survey will be discussed at the next PRG Meeting and an action plan will be developed based on the feedback we receive. A summary of the findings will be published on the practice website.
