Your Patient Participation Group is working with the Medical Centre to find out what people think of our services and how we can improve it.
Please help us by completing this questionnaire and returning it to us; it only takes a few minutes. Please note this questionnaire is also available in paper form from reception. All replies are anonymous and confidential.
The results will be published on the Patient Participation Group’s notice-board and on our web-site. There is an area on the last page for you to add any additional comments. These will be added to the PPG’s ‘You Said, We Did’ report and placed on the notice-board.
