Patient Access - evening and weekend appointments

We have been asked by NHS ENgland to provide an element of clinic time outside of our core hours of 8.30am - 6.30pm. Currently we are open on a Saturday morning but we would like to ask our patients if there are any other times they would like to be able to access the services of their GP.

We would be grateful if you would kindly complete the following questionnaire to give us an indication of what opening times you would prefer.

In addition to our normal working hours of 8.30am - 6.30pm Mon - Fri, are there any other times you would like to be able to access the services of your GP? Please select one of the following answers:
