In order to improve the services we provide, we would like to ensure that our patients understand when, and by what means, their data is being shared.
We are obliged to share the information in accordance with the law and in line with the expectations of the public.
We would like to use this brief survey to assess that our patients understand their consent choices and feel that their wishes are respected.
