Alvanley Family Practice Patient Survey 2013 - 2014
Dear Patient
As part of our on-going commitment to improve and develop the practice for all our patients, we would be very grateful if you would complete this survey about Alvanley Family Practice.
We recently ran a mini survey of the priority areas that patients would like us to focus on. We are always looking for more people to join our 'virtual' patient reference group, whereby patients can share their views with the practice via email discussion. If you are not already part of this group and would like to join, please leave your email address and name in the box in question 23.
Please answer all the questions that apply to you and remember, there are no right or wrong answers, and everybody's view is important.
