Sherbourne Patient Survey 2014
Sherbourne Medical Centre and your Patient Participation Group are undertaking a survey jointly to find out your views on some of the services provided at the practice. We undertake this annually and previous results can be found on the practice website (http://www.sherbournemedicalcentre.co.uk/) or on the noticeboard in the waiting area. This year we have decided to focus on the appointments system.
As part of this we also want to find out how much people know about how the existing system works so we can check how well our communication about this works. So we also have a short 'quiz' to test your understanding of the existing system (sorry no prizes!). Full details about the system are avilable on the website or by speaking to the reception staff.
Quiz

I can make an evening appointment with a GP if I work during the day

I can only make appointment with a GP by ringing or coming in on the day
I can make an appointment up to 2 weeks in advance
