Patient Survey 2013-2014
We are conducting a short survey concerning the choices our patients make in seeking medical help at the surgery or attending an Accident and Emergency Department (A&E). We would be very grateful if you could spare the time to complete it.
1. Have you or a child that you care for attended A&E in the last year?
2. Did you attend A&E during surgery opening hours (between 8am - 6.30pm)?
3. Did you try and get an appointment at the surgery or ask for advice before attending A&E?

The surgery has release on the day GP appointments everyday if you phone at 8am?
Additional slots for urgent care are available at the end of morning surgery and in the middle of evening surgery?
You can book GP and nurse appointments at least one month ahead?
