THE BOULEVARD MEDICAL PRACTICE SURVEY - November 2013

Thank you very much for agreeing to take part in our annual survey to help our practice look at ways of improving. The survey this year is also designed to help feed into the Calderdale Health Forum which now meets regularly.

The Calderdale Health Forum has been set up to collectively involve local residents, voluntary and community groups and patient representative groups who care about the NHS. The forum will act as a network which will help the Calderdale Commissioning Group (CCG) to gather patient's views, through surveys, focus groups and conversations. The CCG can then help to make services more responsive to patient needs. The CCG will also use the information to help make decisions about planning new services locally.

Please answer all of the questions and click 'Send Survey' when you are done if you are completing online. Otherwise please pass or post the completed paper copy to reception.

The survey has been put together with the help of the practice's Patient Group. Once sufficient responses (minimum of 250) have been received they will be collated and reviewed with the patient group. If you are interested in being involved please advise reception or keep an eye on the website for the date of the next meeting.
