Q1: Thinking about the last time you contacted the practice by telephone. How satisfied were you with the manner in which the receptionist handled your request?
Q2: If you were booking an appointment how satisfied were you with the choice of appointment times available to book?
Q3: Thinking about the last time you visited the practice how well did the receptionist greet you when you arrived?
Q4: How well did the receptionist respect your privacy and confidentiality given the limitations of the reception area?
Q5: How well did the receptionist listen and understand your requirements?

Q7:Thinking about the last time you collected a prescription from reception. Was your prescription ready when you came to collect it? (n.b prescriptions are to be available within 48 hours of the patient's request).

Q9.When you collected your last prescription were the items listed as you expected?
Q10:When you collected your prescription from the chemist were the contents of the bag the same as listed on the prescription?
Q11:Are you aware of our Minor Illness Service provided by our Nurse Practitioners?

Q12:Would you choose to see a Nurse Practitioner for a minor illness problem rather than a GP?
Q13:If you have recently consulted with one of our Nurse Practitioners did you feel your problem was resolved?
