Welcome to our patient survey. The purpose of this questionnaire is to gather patient’s views on the services we provide. It is important for us to ensure patients feel involved with the surgery, and the decisions we make on commissioning services for our patients.
We encourage active engagement through our PPG ‘The Friends of Oaklands’ and would like to extend this to our online users as well.

We have an active Patient Participation Group ‘Friends of Oaklands’ to which all patients are members. We are keen to encourage more patients to join this group so that it better reflects our practice population – young and old. There are regular coffee mornings the first Monday of every month 10.00am-11.30am upstairs in the surgery alongside the evening meetings on Wednesdays 7.00pm-8.30pm (approximately every 6 weeks). See the timetable on the PPG notice board in the Stade Street entrance. We are keen to extend our group to our online users – if you would like to receive information/questionnaires/newsletters on line please complete the ‘Friends of Oaklands’ registration form on the website or be part of our virtual Patient Participation Group
