This anonymous questionnaire has been built with the help of our Patient Reference Group. We are again seeking patients' views on some of our services. All the responses will be fed into a summary report which will be available for our patients, in the surgery and on our website from early January 2013. We will look at developments/improvements we can make from this survey.
