Dear Patient,

Many thanks for taking part in this short survey. Your answers to the questions will help our practice understand how we can improve, if at all.

Please answer all of the questions and click 'Send Survey' when you are done.

Q1. How easy do you find getting into the building at the surgery ?

Q2. How clean is the GP surgery ?

Q3. In the Reception Area, can other patients overhear what you say to the receptionist?

If this is a concern to you, how would you suggest it is changed?

Q4. How helpful do you find the receptionists at the surgery ?

Q5. How long after your appointment time do you normally wait to be seen?

Q6. How do you feel about how long you normally have to wait ?

Q7. Many patients attend wanting advice on multiple problems in a 10 minute appointment, do you feel the clinicians should stick to the 'one appointment, one problem' approach when they see patients in order to keep to time?

Q8. Did you know you can request a double appointment when booking an appointment if you have more than one medical problem to discuss?

Q9. Do you feel that a 30 minute+ wait is acceptable if clinical staff are having to deal with a clinical emergency that has presented in the surgery?

Q10. Have you visited the practice in the last 6 months?

Q11. How do you prefer to book your appointment?

Q12. The practice has four lines taking calls throughout the day. If you regularly call the surgery you will be aware that the opening part of the day is very busy with calls for patients wanting treatment for that day which is why we try to encourage those wanting non-urgent matters to call after 10am. On average how many times do you have to call to get through to the practice?

Q13. If you have difficulties calling through to the practice due to the lines being busy, at what time of day would you say this is?

Q14. Lastly, we try to ensure as many of our patients know about changes as they happen. How do you find out about changes to the practice?

Do you find these informative and if not what would you like included?
