New Service Proposal
Following on from a survey, undertaken last year, the Patient Participation Group have put together a design for a new service for patients at Cranleigh Gardens Medical Centre and we would like to ask you for your views on this proposal.The service is aimed at helping patients to:
a) Find out more information about their health and any recently diagnosed problem, helping them to find support, talk to others with the same issues, join a long term condition or disease related group and to learn how to cope effectively with a new health problem.
b) Find the answers to more general health questions that a patient may not want to bother the doctor with but where they may not know who else to ask.
For example, how do I find out how to leave my body to science, or where can I get a private medical for an Australian visa application?
The service would be available in three parts
1. A dedicated computer terminal at the surgery available for patient’s use which would be specifically linked to all the main accredited disease/ long term condition websites, support associations and NHS help and guidance. A member of staff would be able to help patients use the computer, or do a search for them if the patient is not confident with using /IT equipment.
2. A dedicated information help-line for patients, separate to the surgery lines, which will be answered by a member of staff who will be tasked with finding out the answers to any queries raised. Queries could also be raised by email.
3. A series of small facilited self-help groups aimed at bringing people, who have the same concerns or health problems, together to learn from each other, to understand their condition better and to explore ways of managing it more effectively.
QUESTIONS
