This questionnaire asks what patients think of their surgery, how they consider the services and what they think of the quality of service.

It helps us if we know why you need an appointment and so our receptionist will ask you for a reason. This is to ensure that your appointment is with the appropriate clinician and for the appropriate length of time. Do you think it is acceptable to be asked?

Music is often played in the waiting areas for background noise and to afford confidentiality to patients in Consulting Rooms. Do you agree with this?

The surgery receives a lot of information that would benefit patients, but it is difficult to display everything in the waiting room. How do you think we could best make this information available to patients?
