Bury Knowle Health Centre Patient Survey 2012

In August 2012, as part of our focus to improve patient access and continuity of care, Bury Knowle introduced Same Day Access to named doctors. This means when a patient contacts the surgery, the doctor that they usually see (their 'named doctor') will call them back within a few hours to give advice or arrange for a mutually convenient time for them (the patient) to come to the surgery for an appointment - often on the very same day.

We would value your feedback on what you think of the new system and other aspects of the practice.

Please tick the most appropriate box for each question and write in the larger boxes as directed.

Your response is completely anonymous and your doctor will not know how you have answered.

This questionnaire refers to the consultation you have just had.
