The Old Dispensary Patient Participation Group 2011/12 Patient Survey

Dear Patient - many thanks for agreeing to take part in short, occasional surveys to help our practice to gather ideas for improvements or changes to the services we provide. These surveys also help us to check that the quality of the service you receive is of a high standard.

The following survey should take no more than 5 minutes to complete and we look forward to receiving your reply. The areas covered have been decided by analysing the results of questionnaires from a cross section of our Patient Group.
