The Patients Group have recently carried out a survey of 63 patients in the waiting room to ask for their opinions about the practice. Several patients had the same concerns and we would like to give more patients the chance to comment. All staff at the practice are committed to working with the Patient Group to improve services and to provide the highest standard of care. Feedback from this survey will help us all to identify areas that may need improvement. Your opinions are very valuable.
Please answer ALL the questions that apply to you. There are no right or wrong answers and your doctor will NOT be able to identify your individual answers.
Thank you.
