Birchwood Surgery Patient Survey 2011/12

This survey has been developed in association with the Birchwood Surgery Patient Reference Group and is part of the Department of Health’s initiative to increase patients’ involvement in the services provided by their practice.

Please fill it in honestly, the survey will be anonymous and will in no way be detrimental to the care you are given in the future. Please try and express any comments constructively as one of the purposes of the survey is to identify areas in which we could do better. Comments will not be replied to individually, if you would like to raise a complaint or a compliment that requires a reply this should be done in writing addressed to the practice manager.
