Over the last few months patients involved in the Patient Participation Group have devised the following questionnaire with input from the surgery. This has been based on previous national questionnaires but has been changed to take in to account local issues. Please answer the following questions and either post back your copy to the practice or leave it at reception. The questionnaire is entirely anonymous.
